St. Martin’s Parents:

As we prepare for the 2021-2022 school year, I am sending information regarding health forms. Some are required while others are optional.
​
The following documentation is required for all students:
All students must have a copy of their updated immunization record on file per state regulations. For the 2021-2022 school year, there are new requirements:
• All K-7 students must have had two varicella vaccinations.
• All 7th and 8th graders must have had one Tdap vaccination and one meningococcal (MCV4) vaccination.
Please see “Vaccine Requirements for Children … ” form for reference. The form is attached.
• Emergency Contact Form needs to be completed every school year.
• Health Assessment I needs to be completed by the student’s parent or guardian.
• Health Assessment II needs to be completed only by a physician or nurse practitioner.
• Blood Lead Testing Certificate is needed for all Pre-K, Kindergarten and 1st grade students.
The following forms are optional:
• Permission to Dispense Over-the-Counter Medications o This form grants us permission to dispense medications that we keep a stock of for your student. This form must be signed by a parent/guradian and a healthcare provider. No over-the-counter medication will be administered without this form completed. This form needs to be completed every school year.​
• School Medication Administration Authorization Form o This form is for medication that needs to be administered daily or on an as-needed basis (inhalers or EpiPens) at school. This form is to be completed fully and signed by a parent/guardian and a healthcare provider. A new form must be completed at the beginning of the school year for each medication and each time there is a change in dosage or time of administration.​
